The method of D-dimer quantification in the human blood plasma has been developed using monoclonal antibodies 111-3b and II-4d. The method has been verified on the blood plasma of the patients with ischemic heart disease with and without stenocardia and with hypertension. The results showed that at ischemic heart disease with and without stenocardia and at hypertension the quantities of D-dimer in the blood plasma were generally less than the highest normal level 500 ng/ml (64.3%, 76.2% and 95%, correspondingly). The semiquantitative measurements of soluble fibrin levels in blood plasmas of the patients with ischemic heart disease and hypertension have been performed. It has been shown that the quantity of soluble fibrin at these diseases range greatly from < 0.03 mg/ml to 0.15 mg/ml. There was no correlation between the quantities of D-dimer and soluble fibrin in blood plasmas of the patients. Electrophoresis in PAAG with SDS showed that the soluble fibrin at these diseases had the mo- lecular mass of the fibrin (ogen). Thus the soluble fibrin in blood plasmas analysed consisted mainly of fibrin desAA oligomers (may be with fibrinogen incorporation) which are not stabilized by the factor XIIIa.

A study of 68 patients showed that the ergometrine test is a rather informative method of the diagnosis of ischemic heart disease that supplements essentially the results of loading tests. The authors demonstrate the possibility of using the ergometrine test with the purpose of evaluating the effect of antianginal therapy.

Relationships between myocardial blood flow, myocardial contractility, anaerobic metabolic products, and myocardial oxygen requirement and uptake were investigated in 52 coronary atherosclerotic patients. Myocardial functioning under the conditions of inadequate myocardial perfusion, where blood supply requirement, determined by myocardial activity, exceeds actual magnitude of coronary blood flow, is shown to be basically possible in patients with marked atherosclerosis of coronary arteries. Specific features of myocardial performance under the circumstances are its partially reduced effectiveness and activated anaerobic carbohydrate oxidation pathway, as evidenced by a negative arteriovenous difference between lactic and pyruvic acid levels in blood from the aorta and the coronary sinus.

Variation in cumulative electrocardiographic depolarization (delta epsilon QRS) and repolarization (delta epsilon T and ST depression below 1 mm and more) parameters at threshold stress, achieved by stepwise augmentation, was compared with perfusion pressure in the left coronary arterial network, measured at coronaro-angiography, in 103 coronary patients. It is concluded that changes in cumulative ECG parameters at threshold stress can only be suggestive of a possible drop in perfusion pressure in the left coronary arterial network.
